Consent to Disclose Academic Records

In keeping with the Family Educational Rights and Privacy Act of 1974, as amended, UW-Extension will not release academic records without prior written consent of the student.

If you need us to disclose information about your records to a third party, please complete a consent to disclose form. You will receive this form with your statement of account after you register in a course. You may also request this form from our office. On the consent form, you will need to indicate whom you wish to receive the information and what information you wish to release.

There are certain instances in which no consent is needed. These may include requests from Independent Learning faculty and staff with a legitimate educational "need to know"; requests in accordance with lawful subpoena or court order; requests from representatives of agencies or organizations from which you have received financial aid or guaranteed student loans; requests from officials or other educational institutions in which you intend to register; and requests in connection with an emergency if the knowledge of such information is necessary to protect the health and safety of the student or other persons.

UW-Extension will release the following directory information upon request unless you state in writing that it not be released: "your name, address, telephone, date of birth, major field of study, dates of registration, degrees and awards received, most recent previous educational agency or institution attended, participation in officially recognized activities, and (for athletes) weight and height."
